How We Help Our Patients

The Palliative Care Service at Stony Brook Medicine serves as a vital lifeline for individuals facing serious illness at Stony Brook University Hospital (SBUH). Our mission is to provide compassionate, holistic care that improves the quality of life for patients and their families during the most challenging times, including at the end of life. We focus on alleviating suffering, managing pain and addressing emotional, spiritual and social needs, ensuring that patients experience comfort and dignity throughout their illness.

Our specialized team — comprised of doctors, nurses, social workers, chaplains and volunteers — works collaboratively with patients and their families to create personalized care plans that prioritize comfort, communication and choice. Our goal is to empower patients to make informed decisions about their care, reducing anxiety and fostering a sense of control during uncertain times.

How You Can Help Our Patients

• Patient Travel Fund: Supports patients at end of life who wish to be reunited with loved ones out of state or abroad but lack the financial means to do so.
• Medication Fund: Covers medication costs and delivery fees for underserved patients or those facing financial hardship.
• Transportation Fund: Provides transportation assistance for families wishing to visit their loved ones who are hospitalized at Stony Brook University Hospital and are at end of life.
• Bereavement and Grief Support: Supports the creation of a memorial ceremony and/or garden to honor patients who have passed, and funds supplies for legacy-making and memory keepsakes for families, including children.
• Education Fund: Provides financial support for continued education for aspiring hospice and palliative medicine specialists and others seeking to integrate palliative care into their field, including medical students, physicians in training, and undergraduate students. This includes funding for conferences, webinars, courses, and other educational opportunities.
• Volunteer Program Expansion Fund: Supports orientation and educational resources for palliative care volunteers, as well as coping and comfort resources for hospitalized patients.
• Research and Community Outreach: Supports the Stony Brook Palliative Care team’s efforts in research initiatives and community outreach programs
