You must complete this form if you wish to have your own official fundraising page during 40 for Forty, but please note that filling it out does not ensure your participation in the campaign. In order to participate, you will be required to have a UT account you can fundraise into or be fundraising on behalf of an area of campus that has a UT account. We will contact you if you are ineligible to participate. Primary contact first/last name and email will be taken from the top of the form. Primary contacts will serve as the point of contact between your group and the 40 for Forty team - this is the person who will receive all official 40 for Forty emails and will be the first point of contact for questions from the 40 for Forty organizers. When available, they will also receive access to 40 for Forty reports and to the GiveCampus platform to make edits to your organization's page.
Secondary contacts will serve as a second point of contact between your group and the 40 for Forty team. When available, they will also receive access to 40 for Forty reports and the GiveCampus platform to make edits to your organization's page.
Please choose one that best fits your group. *We may re-assign your choice if it aligns better with a different place of impact.
If you are unsure if your designation exists or is not available on the online giving page, please contact your CSU’s development staff ASAP to request that these designations be added to the online giving page no later than March 28, 2025. If you are unable to submit this request, please contact Annual Giving Programs at 40forForty@utexas.edu.
